Uh....I dont think they count as nightmares anymore if you enjoy them...doesnt matter how twisted they are...if you like um they arent nightmares XD
But yeah if nothing is helping then you need outside help...clearly none of us are specialists...and I realize you dont wanna be on meds but it seems like your last option is to opt for professional help here.

Being in the nut house with a couple of schizo's and having a cousin who is also schizo, I recon you sound like you have the symptoms Cheesy. Go to a doctor (GP), get them to refer you to a psychiatrist, and go from there.
If it is scitzophrenia you will definitely have to go on meds buddy, sorry to tell you. Better that, though, than letting the voices take over and burning down a school or something.
Oh, and see your GP by yourself. Go alone and you will be able to be more honest, which will help in the long run.
