It IS mentioned in the game. In the part after you've met the President of Esthar city, which is Laguna, you get onboard the Ragnarok. And instead of going up to the top floor where the control room is, you go just around the corner where Laguna and his buddies are. You talk to the buddies first and from Watts you learn that Laguna is his father.
Now, regarding the LIONHEART-issue. There is nothing indicating that his surname is lionheart - in the game that is. But in the manual- book of VIII you can read the profiles of each character very throughly written

I think that Squall just sorta chose his own surname.....either that or Leonhart might have been Raine's surname....

Well his Limit is Lionheart so maybe he chose it because he really likes the move or sumthin
