...did you play fallout? ONE vault in the entire game stayed perfectly intact, all of the other vaults were compromised in some way or another, either by random beasties and/or crazy psychopaths.

Actually, most vaults were compromised because of:
a) Residents inside becoming super mutants, riots whatevs.
b) Fault with the vault, necropolis not having the door close intentionally ^^
